Dallas 911 Not Responding

The City of Dallas has multiple questions to answer about the Dallas 911 not responding after a deaf woman, Zarea Dixon, was found dead in her home when police officers took over an hour to respond to her call for help.

In a separate incident it took more than six minutes to respond to an apartment fire in North Dallas.

Dixon called 911 to report her boyfriend had broken into her apartment along South Polk Street and attacked her with a knife.

An interpreter with Sorensen Translation Services relayed that Dixon had been attacked stating her “ex-boyfriend broke into her house, beat her up and tried to stab her with a knife.”

Dixon provided the suspect’s name, description and date of birth to the police through the translator. She also said he had left her home.

Dixon “declined an ambulance but stated she ‘needed the police,'” according to the affidavit.

In addition to the non response to this woman, Dallas Police has not responded to multiple other incidents.

Additionally, Dallas police never took a report on three students burned by fireworks launched at them in North Dallas.

At another event, sixty to seventy firefighters responded and saw flames coming from the second floor of the three-story apartment building upon arrival last week.

The fire quickly spread to the third floor and then into the attic space and roof of the building requiring the additional alarms by Dallas Fire Rescue.

Dallas Fire Station 29 is approximately one mile away or roughly three minutes.

In comments to the media, Dallas Fire Fighters Association President Jim McDade said, “When the first companies got there — there was an enormous amount of fire, probably due to a delay in response,” McDade said.

There were no deaths or injuries, but more than twenty apartments were destroyed and more than one hundred residents displaced.

Slow responses can endanger firefighters. In New York, a firefighter with Brooklyn’s Ladder 170 lost his life last weekend.

Two men were arrested in Washington, DC for posing as federal agents. The situation may have been a direct threat to First Family.

On Wednesday, April 6, 2022, federal prosecutors charged Haider Ali and Arian Taherzadeh saying they posed as federal agents. It appears they broke U.S. Secret Service security protocols providing agents with apartments, iPhones, drones, and a gun case.

One agent is reported to have worked on the protective detail of First Lady Jill Biden.

In court documents, prosecutors allege Ali, 36, and Taherzadeh, 40, had falsely claimed to work for the Department of Homeland Security. They claimed to be working on a special task force investigating gang and violence connected to the January 6th insurrection at the United States Capitol. They allege the two posed as law enforcement officers to integrate with actual federal agents.

Federal prosecutors said four Secret Service employees were placed on leave earlier this week as part of the investigation.

This news comes as Members of Congress continue their investigation into the attack on the US Capitol which included participation by former FDNY firefighter Thomas Fee.

It is unclear how far this investigation will stretch, but it brings up other internal planned and attempted attacks against the United States by those sworn to protect it. Last year, an active duty soldier was arrested plotting to bomb the 9-11 memorial.

An off duty NYPD DWI charge is the latest embarrassment for Mayor Eric Adams and Officer Tomas Garcia.

Garcia, a thirty-one year old, was sleeping in his personal vehicle at Eighth Avenue and 61st Street when he was woken up by EMS workers around 8:30 AM.

The off duty Garcia was showing signs of intoxication and arrested at the scene.

The officer was charged with driving while intoxicated per police records.

Officer Garcia was in the 66th Precinct at the time of his arrest. It is unclear if he has been reassigned or still working patrol of any kind.

Two Pennsylvania State Troopers were killed near Philadelphia in March. That accident has been under investigation for possible connection to DWI.

Dallas Mom Arrested In Road Rage Shooting

A Dallas mom, Lacravivonne Monique Washington, has been arrested after she reported her three year old son had been shot in a road rage shooting for outstanding warrants per Dallas local news.

In addition to the current charges, Dallas Police have charged Washington with Child Endangerment and Tampering with Evidence. The investigation determined she concealed a handgun during the investigation and that the three year old had access to it.

Monday morning, Washington arrived at Medical City Children’s Hospital Emergency Room with her three year old son who had been shot.

She reported the child had been shot during a road rage incident. The child died of his injuries at the hospital.

Detectives have pursued leads, but have found no evidence of a road rage incident.

There have been other road rage incidents leading to shootings, but per one Dallas police source this appears to be an accidental shooting by an unsecured firearm. We will update news on this story as it becomes available.

Freeport EMS Fired Their EMS Director

Freeport EMS fired their EMS Director, whistleblower Chris O’Leath, after he found sixty-two incidents of financial irregularities totaling nearly $100,000.

O’Leath audited Freeport EMS financial records and found sixty-two unauthorized transactions between January 2017 to May 2020 by the former organization Treasurer.

Mayor Jim Swartz said of O’Leath’s firing: “I don’t care if you’re a director, you still have bosses and when you are told to do things, you do things.”

State Trooper Anthony Vaccaro confirmed that an investigation is continuing. He is waiting for the results from a third-party forensic audit of the EMS’s books, he said.

In the 2021 letter to local media, O’Leath accused Justin DeAngelis, then a former treasurer of the EMS and Freeport Volunteer Fire Department, of stealing nearly $98,000 and then repaying the ambulance service.

AMR paramedic Jason Dean Anderson accused of groping teenager who was his patient on an ambulance transport.

The 22-year-old American Medical Response ambulance medic is accused of groping and soliciting sex from a teenage girl he was transporting to a hospital. He was released from jail Saturday after posting a $75,000 bond.

Deputies from the Riverside County Southwest Sheriff’s Station began investigating allegations that an AMR medic had sexually assaulted a teenage victim and propositioned the victim for sex acts during a medical transport call.

The sheriff’s office did not specify the victim’s gender or age, why the victim was being transported, or where the victim was being transported to given the sensitive nature of the investigation.

The Riverside County Sheriff’s Department believes there may be additional victims.

Law enforcement asking people to come forward and anyone who believes they may have been victimized by Anderson is encouraged to contact Investigator Wesley Martinelli at the Southwest Sheriff’s Station at (951) 696-3006.

Eugene Paramedic Edward Blake Sentenced

Former Eugene paramedic Edward Blake was sentenced to twenty-five years in prison.

Blake, a former paramedic with Eugene-Springfield Fire, was sentenced for rape and sodomy charges. The guilty verdict came down January 31st, 2022. His conviction relates only to a single victim though there were questions about whether Blake was serial rapist.

In addition to registering as a sex offender, Blake will serve prison sentence plus eleven years and eight months in supervised release.

During his trial, evidence was presented showing he used EMS drugs including ketamine, fentanyl and morphine to sedate his victims.

Officials say one woman told investigators Blake told her he was a Eugene Springfield firefighter and sent her photos of medical supplies and medications he claimed to have taken from an ambulance. The woman said she went to a motel with Blake, was choked unconscious and regained consciousness to find an IV in her arm. Blake is accused of drugging the woman with fentanyl and sexually assaulting her while she was unconscious.

Another woman told investigators she obtained controlled substances from Blake and was hospitalized multiple times, and that Blake would come into her hospital room and administer drugs to her through an IV.

“She recalled Blake telling her to sell the drugs, but warned her not to let anyone keep the vials, because they could be traced back to him,” the U.S. Attorney’s statement read.

Michael Greenshaw Sentenced

Michael Greenhaw, 43, a former Alabama paramedic was sentenced Tuesday for stealing from more than 100 vials of fentanyl and replacing their contents with saline.

Greenhaw, was ordered by a federal judge to serve fifty-one months in prison after pleading guilty in October 2019 to one count of tampering with a consumer product. Greenhaw, while working as a supervisory paramedic at First Response Ambulance Service, removed the contents of 129 fentanyl citrate vials for personal use and then refilled the vials with saline, according to a plea agreement. The thefts took place during Greenhaw’s shifts between April and August 2018.

“Greenhaw put vulnerable patients in grave danger by replacing fentanyl citrate with saline in vials that he knew were intended to be administered to critically ill or injured patients,” U.S. Attorney Prim F. Escalona said in a statement. “Greenhaw’s criminal conduct undermines the tireless efforts of first responders who work hard to make sure that patients receive the necessary emergency medical care.”

The case was investigated by the U.S. Food and Drug Administration Office of Criminal Investigations Miami Field Office, the Decatur Police Department, and the Arab Police Department, with Assistant U.S. Attorney Mary Stuart Burrell serving as lead prosecutor.

A Corvallis firefighter resigned during an investigation of their use of department narcotics.

Ogden Ambulance Theft

An Ogden ambulance theft occurred early Sunday. The emergency vehicle was stolen from the area of 3rd Street and Washington Boulevard around 2:50 a.m., the city said in a statement.

An Ogden Fire Department crew was responding to a medical emergency in the area. When they exited the building to transport the patient to the hospital, the ambulance was gone.

Ogden Police shared a post Sunday morning about the theft. Around 8:30 am a tipster reported that they saw the ambulance in a neighbor’s back yard on the 500 block of 7th Street.

Authorities recovered the stolen ambulance early Sunday morning in Ogden. They found the vehicle was not damaged and no equipment was taken.

“Although the recovery was a success, the theft of the ambulance compromised patient care. The delay in patient care while waiting for a second ambulance to arrive could have had negative outcome for this patient in need,” Ogden City wrote. “Driving an emergency vehicle requires training and when driven improperly creates a dangerous situation for the unknowing citizens who share the roadway.”

Police are investigating, but there are no current suspects or motives for the ambulance joyride.

In other weekend news, an AMR ambulance erupted in flames in Washington State.

Jeffrey Sanford Jr.

Jeffrey Sanford Jr., a former central Illinois paramedic, has been convicted of sexually assaulting a patient in an ambulance has been sentenced to 13 years in prison.

On Tuesday, Judge Randy Rosenbaum said the paramedic destroyed the public trust in calling for an ambulance when choosing a sentence for Sanford.

In October Rosenbaum found the 48-year-old Jeffrey Sanford Jr., guilty of two counts of sexual assault in October after forcing a suicidal woman to perform oral sex in the back of an ambulance in July 2019. Judge Randy Rosenbaum noted when convicting Sanford that the victim had immediately reported the assault to hospital staff and that Sanford’s story had changed after DNA evidence tied him to the crime.

Champaign County Assistant State’s Attorney Kristin Alferink also presented evidence linking Sanford to sexually inappropriate conduct with a 10-year-old girl, a woman who called for an ambulance and an 89-year-old woman with dementia. Sanford’s defense attorney, Anthony Bruno, acknowledged his client’s faults but argued for a lighter sentence based on Sanford’s more than 20-year career as a paramedic, adding that he had saved 25 cardiac arrest patients and delivered 36 babies.
